318.750 - Maternity Allowance Application Form

In the interests of a prompt assessment of your application, we kindly request that you ensure that the information you provide is accurate and complete. Please mark the appropriate box.

You should submit the completed and signed application form, together with all necessary accompanying documentation, to:
  • your employer if you are a salaried employee
  • the compensation fund in charge of the collection of contributions or the cantonal social security office, if you are self-employed
  • your last employer if you are unemployed
  • to a compensation fund if none of above three categories apply to you
