318.371 - Survivors pension application form

In the interests of a prompt assessment of your application, we kindly request that you ensure that the information you provide is accurate and complete. Please mark the appropriate box.

Please note that the rate of the survivors pension will only be calculated once the insured risk materialises. The following information is used to calculate the pension: the creditable contribution years, the income earned by the deceased, as well as any educational and child-care credits.

The present application should be submitted to the compensation fund into which the deceased had last paid AVS contributions. A list of addresses can be found at www.ahv-iv.ch
